Our in-depth interviews with 19 stakeholders residing in Melbourne, Australia, uncovered a fundamental alteration to commuter travel habits during the COVID-19 work-from-home era. Participants generally agreed that a hybrid work model would follow the COVID-19 era, typically including three days of office work and two days of remote work. Our analysis of work-from-home influences revealed 21 attributes, which we then distributed across the five standard SEM levels (intrapersonal, interpersonal, institutional, community, and public policy). A further addition to the hierarchical structure involved suggesting a sixth, higher-order, global level to reflect the wide-ranging worldwide effects of the COVID-19 pandemic and the computer programs that enabled work-from-home initiatives. Emergent repair for acute type I aortic dissections was performed on 120 patients (70% men, mean age 58 ± 13 years) during the study timeframe. Of the 41 patients studied, 34% encountered acute ischemic complications. In the analysed dataset, 22 patients (18%) showed leg ischemia, 9 (8%) experienced acute stroke, 5 (4%) had mesenteric ischemia, and 5 (4%) had arm ischemia. Persistent ischemia persisted in 12 of the 100 patients (10%) who underwent proximal aortic repair. Seven patients experienced persistent leg ischemia, one had intestinal gangrene, and one patient required a craniotomy due to cerebral edema; these nine patients (eight percent) required additional interventions. Three additional patients, having undergone acute stroke, manifested permanent neurological deficits. Even with mean operative times exceeding six hours, the proximal aortic repair enabled the resolution of all other ischemic complications. When comparing patients with ongoing ischemia to those whose symptoms ceased following central aortic repair, there were no differences in demographics, the extent of the dissection in the distal region, the average operative time for aortic repair, or the need for venous-arterial extracorporeal bypass support. Six of the 120 patients, or 5%, unfortunately, experienced death during their perioperative procedures. Among 12 patients experiencing persistent ischemia, 3 (25%) succumbed to hospital-related causes; conversely, none of the 29 patients whose ischemia resolved following aortic repair died in the hospital (P = .02).
